II. Overall Responsibility The Social Services Advocate helps bridge the gap for patients and connects them with additional healthcare resources. Primary duties include assisting patients who have been referred to a specialist by the Nurse Practitioner, in completing eligibility paperwork for UAB Charity Care or Project Access. The Social Services Advocate is also responsible for scanning documents, including eligibility applications and medical records, into the Clinic’s EMR. III.
